Becoming a patient with us is super easy, but there are a few steps to get you started. Don’t worry—we’ll walk you through it all!
Step 1: Fill Out the Form
Click the link below to access our prospective patient form. Pro tip: Fill it out completely, including your Insurance Member ID number. Don't just take a picture of your card—your insurance verification is like a VIP pass, and our clearing house needs the details typed out to work their magic.
Step 2: We Review Your Info
Once we get your completed form, we’ll review it. If you’re new to the practice (and to our providers), you’ll get a call from us to ask a few follow-up questions—things like what you’re looking for and your current diagnosis. This is our way of making sure we’re the right fit for you!

Once we’ve got everything sorted, including if you're following your prescriber, you’ll receive 8 consent forms via email. We promise they aren’t scary, just the usual legal stuff! Once you sign those, it’s time to move to the next step.

You’ll get a text from us (or an email if you prefer) with a code to activate the MYIO portal. Don’t worry—it's not a secret code for a treasure chest, just the key to scheduling your appointment.
The MYIO portal is the gateway to paying any co-pays, checking in for appointments, virtual appointments, medication refills, appointment request, and messaging with your provider and other staff.

Once you activate the portal, we’ll schedule your appointment, and you’ll be one step closer to joining the Psych Alchemy family!
The MYIO portal is needed to pay any co-pays, check in for virtual appointments, and where virtual appointments take place.
