Self pay rate is $300 for initial visit. Follow up visits are $150.

- Blue Cross Blue Shield (Anthem, FEP)
- Aetna (Meritain)

- Medicare
- Tricare
- United Health Care
- If you don't see your insurance listed, please give our office a call to confirm if we accept it.
- If we do not accept your insurance, you are still welcome to be seen as a self-pay patient.
- Please know that all payments are made via debit/credit card on the patient portal. Cash and check are not accepted.
